When I use the S key in the "save as" window rather than clicking the Save button, I am prompted that the file already exists even though it didn't exist before pressing S.

PMView 2000 for Windows, Version 2.14
Windows 2000 Professional

I can't repeat this problem.

Can you please give me a step-by-step example that shows how to repeat the problem. If only possible, please use a captured image and an empty target directory in your example.

This is what I did:
File->Capture->Window
File->Save as
Press the 'tab' key until the [Save] button is active
Press the 'S' key.

The file saved normally. I can't see the problem.

This is what I did:
File->Capture->Window
File->Save as
Press the 'tab' key (3 times) until the tree view is active and use up/down arrow keys to select a directory to save to.
Press the 'S' key while still having the tree view active.

It also seems that the directory does not change as I would expect when you navigate the tree view like this.

If I do everything the same except I 'tab' over to activate the save button, then press 'S', the directory change is made and no overwrite warning message is given...

Yes, now I see the problem! The keyboard definitely does not work correctly when the tree view control has the focus.

This will be fixed in v2.20 (available late February). In the meantime, don't use the keyboard while the tree view control is active. If you tab to any other control, the keyboard will work fine.
